Direct Deposit Enrollment

Follow the steps below to register for direct deposit. Once processed, Paychex will automatically deposit funds into your bank account on the 15th and last day of each month.

  1. Fill out these three forms in blue or blank ink:
    a. New Employee Information Sheet
    b. Federal W-4 Form
    c. Direct Deposit Enrollment Form

  2. Scan and email them to
